Volunteer Application

The Lake Washington School District requires that every volunteer be screened and cleared by the Washington State Patrol.  Volunteer approval is valid for 2 years. After 2 years, you must reapply.

There are three ways to complete your application:

1.    Online through Parent Accessoption for parents only

The fastest way to have your application processed is to complete the online form through Parent Access. You will need to have an electronic copy of your drivers license available to upload to the application in order to complete this process. If you do not have a scanner, you can take a photo of your license and upload that copy.

2.    Print out a PDF Version of the Applicationoption for all volunteers

A PDF version of the volunteer application from the district’s Web site is available here. (You will need to have the latest version of Adobe Reader installed on your computer in order to open this document.) You can fill out and print this application, then mail it (along with a copy of your driver license) to the volunteer office at volunteers@lwsd.org or via USPS:

3.    Get a paper copy of the application, available at the district’s Resource Center option for all volunteers

The Resource Center is located in Redmond Town Center, at 16250 NE 74th Street, Redmond, WA 98052. You can ask for a volunteer application at the front desk, fill it out and turn in the application on the spot. Please be sure to bring along your driver license so that a copy can be made to go along with your application.
